Send private reports to moderators with a severity level

Members report issues to moderators with /report.

Usage:
- /report — describe the issue and pick low/medium/high severity
- Reports post as embeds in the configured moderator channel with color-coded severity (🚨 high, ⚠️ medium, ℹ️ low)
- The command is private — only the reporter sees their own confirmation

Setup:
Set the moderator logs channel in the dashboard (defaults to #mod-logs in tests).
